Care Management

Live At Home Solutions provides professional care management services in Salina and Saline County, Kansas. You may speak with one of our Care Managers by calling 785-823-1767.

A Care Manager is a trained professional who can help arrange care and in-home services for an older or disabled adult trying to live in their own home. There are several good reasons to consider working with a care manager:

  • Convenience: You get skilled help navigating your way through the confusing maze of services and care options — whether you live across town or across the country

  • Peace of Mind: You get support and advice that is objective, caring, reliable and cost-effective

  • Personal Service: You get personalized assistance that is focused and responsive to your wants and needs

At Live At Home Solutions, our Care Managers offer four stages of service, depending on your needs and wishes:
  1. Initial Screening. You start with a free screening, to see if care management is right for you and to find out if you are eligible for any financial help paying for your care and services. Depending on the outcome of the screening, you may choose one or more of the following services.

  2. Comprehensive In-Home Assessment and Care Plan Development.

  3. First-Month Scheduling Services and Coordinating Care Plan.

  4. Ongoing Monitoring and Coordination of Care.

You will be served by a team of Care Managers who have extensive knowledge about the availability, quality and cost of services in the Salina area. Our Care Managers are compassionate professionals -- licensed social workers and registered nurses -- with years of experience helping older and disabled adults and their families.
