How do I find out if Care Management Services are right for me?
Contact a Care Manager to schedule a free initial consultation. This can be conducted in our office, at your home, or by telephone if you are a long-distance caregiver. During the initial consultation, we will learn more about your needs and wishes and fully explain care management services.
Many times, the initial consultation will reveal that you do not need care management. You may just need a simple referral or a missing piece of information. If that is the case, we will provide the referral or information and you can take it from there. However, if the situation is more complicated or requires assistance that is more in-depth or ongoing, care management may be the answer.
